online survey

Hi,

we want to have an online survey to ask our members about our services, to better sell advertsing on our site. Is there such a script already written that someone could point me towards?

requirements:
an easy installation
easy way to view the tabulated statistics
multiple questions with multiple kinds of answers (radio button, multiple choice etc)
we don't want to members to see the results

I already use a nice online poll called qpoll (which I highly recommend) but it is not quite right. It works well for indiviual questions, but not a comprehensive online survey.

I am a bit green with forms and such. I don't know how it all passed me by...Any suggestions?

I am still confused. I already have poll software on our site and it works quite well. It is called qpoll. (might be called quickpoll) anyway, I really like it for those one-off polls.

But what we want to do is have a poll with like 30 questions. Can these polling softwares do that?

Evie, I really couldn't say it was just something that I remembered seeing one time.  I don't use any polls on any of my web sites at this time.
