login form in page

This might be easy, but here goes... I want to create a login page where the user enters their login/password to enter a site, as opposed to the popup prompt. Can somebody tell me where to find the code and/or scripts necessary for this?

You can try something off of the CGI Extremes site:

Looks like they have quite a few examples there to choose from.
