View Full Version : Forms (Not HTML Kind but images!)

Hey everyone,
I've run into a nice snag. :( Here's my dillemma:
I need to be able to have forms on a clients website, so his clients can print them for themselves. The problem is he has ready ones he uses now, and I just want to scan them (making the image files) available for download or viewing.

The problem? No matter what size graphic I use it prints out distorted? The image? www.kidshealthservices.com/form.gif (http://www.kidshealthservices.com/form.gif)

Any ideas/suggestions/tips/sanity dust/ritalin/prozac?

Bi4Be - this might be a good application for Adobe Acrobat .pdf files??

I hate PDF format badly... but that's me...

I'd either make it a text form or redo the graphic and turn off anti-aliasing on the text, as some printers will make it look fuzzy.

Eeeek! I can't un-anitaliasy thingy this! I only scanned it, and the REAL forms have all these boxes and all.

Pretty stinky if ya ask me, I think I'll strive for mediocre and just scan n' post. :(

Why not make it an HTML form? Why would you require a person to print, fill out, and mail a form? You'd lose my business if only because I don't own a printer (unless you count the dot matrix one out in the shed hehe). Well, that, and I'm too lazy to do something by hand. Do they still make those things - geez, what are they called - Pencil? Yeah, those. Where can you order one of those?

But seriously, I'm not sure what the form is for (didn't really look at it), but I'm sure an online mail form would be much more efficient (not to mention saving all those poor trees hehe :))

At the very least, type it out as a text form - those with dot matrix printers will thank you (yes, there are still a few of us that don't print much of anything). Images on a dot matrix are - well, very ugly.

Just my 2 cents :)

These are medical forms that are required for examinations, not exactly web form applicable. (Why medical forms are on a website is a whole other story, so don't ask *grin*)

And it really isn't the only form of being used, just a timesaver and option to my clients' clients. Instead of my clients having to fax or mail the forms, they can ask if they have web access and a printer and tell them to go download them. Anyways, it's a long story.... :D
