Finally, I have completed the new order forms:
Secure: https://secure.futurequest.net:611/Order/
Non-Secure: http://www.futurequest.net/Order/
You can now order Value Added Services, extra FTP logins, IRM/IRO/Park etc all from the same forms. They also perform a little bit of error checking to help ensure that your orders aren't delayed.
Let me know of any problems at all with the new forms :)
