My Google searches, as well as the Open Directory link that I found in an old forum message, are proving overwhelming. I'm hopeful that someone here has some e-mail list experience that you'd be willing to share, and that will point me in the direction of a better starting place.

I need a hosted email list management solution that will allow me to:

define and store subscriber information beyond just email, first name and last name;
make the subscription form "pretty" (image below); and
easily assign a subset of subscribers to receive a particular email (for example, a random sample of 500 K-5 science teachers on the list)


Does such a thing even exist, and if so, is it (are they) any good (reliable, user-friendly, etc.)?

I see that phpList Hosted (beta) allows me to set the subscriber attributes that I want to collect, and put them on my subscribe form... but what I've tried has resulted in a God-awful ugly form.

I haven't used them, but I've heard Constant Contact is a good system,

Not sure if custom information can be stored with the subscriber information, but might be worth exploring.

Email management is $15 a month for up to 500 subscribers.

Try Mail Chimp. We use if for a large OB/GYN practice with six offices and we have it set up to where we can send emails out to patients of a certain doctor, a practice, by birth year, by zip code, or the whole list. They provide a subscribe form with what you want in it, or you can create your own. Their customer service is great too.
