Sending email attachments

I am attempting to use "Jack's Formmail.php" in a php script to send emails and included an attachment with the email. The php script is called by a form. Although the emails are being sent, no attachments are being sent with them. The file name of the attachment is referenced in the resulting email but the file itself is not sent.

Is anyone using this (or any other script) to successfully transmit attached files with emails? All suggestions and comments are appreciated. Thank you.
