Ok, following up to this thread and waving a white flag dotted with greenbacks:

Solving this problem with my limited skills is getting very frustrating and frankly, is quickly moving into a situation where I can no longer justify the time I'm spending on it.

I need a form built along the specs outlined in the post I've linked to...javascript, php, I don't care. Actually, PHP might be better for future growth such as if we ever want to expand to a small cart in order to offer donor premiums.

It doesn't need any visual customization as I'll be dropping it into our site which is a mambo site. If you want to stick a simple css in there for background and text, that would probably be good but I don't need any more than that with it. I can always put that together if you don't.

Some other issues/thoughts just to save me from doing this again, we are seriously considering getting a secure certificate so I'm not sure if that has anything to do with the form itself other than the location on the server.
