Hi everyone,
I have some projects coming up that will need some cgi (perl or php?) based forms. a similar form is located at http://www.carpal-tunnel-syndrome.com/html/survey.html.

we will be designing a site that needs to be in english and spanish. I need an estimate from someone who could develop forms similar to the one referenced above for a site hosted on futurequest. there will be 3 separate forms, each of which need to be developed in english and spanish. One of our staff does speak spanish and could assist with the process, and no translation will be needed (ie, all text will be provided in both spanish and english). at present i am seeking a bid for simply emailing the data to an account but if you have affordable solutions that would dump the data into a futurequest-compatible database (mySQL i think) I could work that into the plan.

we anticipate ongoing need for such forms (not necessarily in two languages) and have decided not to learn how to do this ourselves so if this contract works out, it could build towards an ongoing relationship or partnership. I would love to find a futurequest community member to fill the slot.

Please email with an estimate by friday, jan 25 of costs for completing this job. the work will be needed in mid to late feb.
